1. Register Your School or Class
This step is required to identify yourself to the system later when you try to retrieve the results for your school or class. It is important to keep a record of these information.

At the main page click on "Register your school/class". On the following page fill in (all are required) the text boxes of the form on the left hand side (see image below) with:

• The name of your school or class. This is an identifier, not necessarily the real name.
• The login name. Any name of your preference.
• Your email address.
• An administrative password. This is the password you use to retrieve the questionnaire results for your school. Confirm this password by re-entering it in the next text box.
• The user password. This is the password that participants in the survey will use to enter your school or class and complete the questionnaire. This prevents any unauthorized person from completing the questionnaire for your school or class. Confirm this password by re-entering it in the next text box.

Register you school by clicking on "Register".

Once you complete the above, tell your participants the school/class name and user password. They will use these information to log on and complete the questionnaire.

The procedure below details how participants can complete the questionnaire.

2. Log on to your school and complete the questionnaire

• On the main page click on "Go to questionnaire". On the following page (see image below) select your school from the drop-down list and enter the password supplied by the principal/teacher of your school then click "Login". On the next page click on "Begin Questionnaire".
• Question number one will be displayed for your response. Continue selecting an answer and clicking next until you reach question number 25 (the last question). A warning will be displayed at question number 25 indicating that you are about to complete the questionnaire. Clicking "Finish" will record your answers and display your average score. Clicking on "Cancel" will provide you the option to go back to the main page without registering your answers.

After some or all participants in the survey have completed the questionnaire, the next step is to retrieve and view the individual and average HR temperature scores for your school/class. You can also view and print individual responses for every question by ID Number. Follow the steps provided below.

3. Log on as administrator to obtain HR Temperature survey results for your school

On the main page click on "Register your school/class". On the following page (see image below) select/fill in (all are required) the text boxes on the right hand side with:

• The name of your school from the drop-down list.
• Your login name.
• Your e-mail address
• Your admin password.

Enter the results page by clicking on "Display Entries".

The entries made to your school will be displayed in a tabular format by ID Number detailing date taken and individual percent score as well as total average score for all participants. A graphical measure of HR temperature is also provided alongside the tablular presentation.

Clicking on ID numbers on the table displays the responses of each participant for each question in the survey. This can be printed and distributed among participants to discuss the points on which poor grades were obtained.
